How to submit a PAIA request
If you would like to request access to information, download the relevant form above, complete it in full, and email it to us. We will confirm receipt and respond in line with the timeframes set out in the Act.
- Download the form you need — Form 2 to request access to a record, Form 4 to appeal a decision. The PAIA Manual explains what records we hold.
- Fill it out completely, including your contact details and the information you are requesting.
- Email the completed form to info@ubuntulife.co.za.

The Promotion of Access to Information Act (PAIA) gives effect to the constitutional right of access to any information held by the State and any information that is held by another person and that is required for the exercise or protection of any rights.
